What the Child and Adult Care Food Program Application Is

The Child and Adult Care Food Program Application is the formal enrollment and reimbursement request used by child care centers, adult day care centers, and sponsoring organizations to participate in USDA's CACFP. It documents program eligibility, meal counts, site and sponsor information, and certifications needed to claim federal meal reimbursements. State agencies administer CACFP funding and receive applications, verify eligibility, and approve sponsors and sites before reimbursement begins. Accurate and complete applications are required to establish participation, set reimbursement rates, and ensure compliance with meal pattern and recordkeeping rules.

Why a Correct Application Matters

Completing the CACFP Application accurately establishes program eligibility and enables timely reimbursement for eligible meals and snacks.

Why a Correct Application Matters

Who Typically Completes a CACFP Application

State agency nutrition offices review and approve submissions, and maintain the official participation records used for audits and payments.

  • Sponsoring agencies managing multiple child care sites and centralizing claims and training responsibilities.
  • Independent child care centers or family child care homes seeking direct reimbursement for meals they serve.
  • Adult day care centers and similar adult care providers that meet program eligibility and meal requirements.

Step-by-step: Filling the Application

Follow these core steps to prepare and submit a complete CACFP Application to your state agency.

  • 01
Collect documents: Gather EIN, licenses, and attendance documentation before starting.
  • 02
Complete form: Enter sponsor and site details, program type, and enrollment counts.
  • 03
Certify accuracy: Sign and date the authorized official section; attest to program compliance.
  • 04
Submit to state: Send the package to the state agency via its accepted channels.

Typical Submission and Review Flow

The application moves through a short review workflow from submission to approval and initial reimbursement setup.

  • Prepare package: Assemble form, licensing, and sponsor documentation.
  • Submit application: Deliver via state portal, mail, or authorized electronic submission.
  • State review: State agency verifies eligibility and documentation.
  • Approval notice: State issues approval and instructs on claims submission.

Digital Workflow Settings to Consider

Configure your digital submission process to match state agency requirements and maintain an audit trail.

Field Configuration
Document format PDF/A preferred for long-term records
Signature type Electronic signature compliant with ESIGN/UETA
Authentication Email + optional SMS code for signer verification
Audit trail Capture timestamps, IP, and signer attribution

Platform and File Requirements for eSubmission

When uncertain, provide both a signed PDF and the native editable file, and follow the state agency's preferred upload or mail instructions.

  • File types: PDF, PDF/A, DOCX commonly accepted
  • Authentication: Email-based signer attribution generally sufficient
  • Retention: Maintain accessible records for audit periods

Security and Compliance Considerations

Access controls: Role-based permissions

Common Preparation Challenges

  • Incomplete EIN or mismatched legal name delays verification and reimbursement setup.
  • Missing site licenses or attendance records trigger state agency requests and slow approval.
  • Incorrect enrollment counts cause under-or over-claimed reimbursements requiring adjustments.
  • Unsigned certifications or missing authorized official details lead to application rejection.

Consequences of Errors or Misrepresentation

Repayment Demands: State may require reimbursement of overpaid funds
Disqualification: Sponsor or site can be suspended from CACFP
Audit Findings: Noncompliance noted in state or USDA audit
Civil Penalties: Financial penalties for false claims
Criminal Risk: Intentional fraud may lead to prosecution
Payment Delays: Incomplete apps delay initial reimbursements

Practical Application Examples

Two typical scenarios illustrate how applications and documentation support program participation and claims.

Example 1

A county childcare network completed a consolidated sponsor application to add five new family child care homes.

  • The network used standardized attendance templates to ensure accurate counts.
  • The result: faster approval and consistent monthly claims backed by consistent documentation across all sites, reducing audit questions.

Example 2

An adult day care updated site licensing and submitted a corrected application during renewal.

  • They attached updated health licenses and an updated authorized official signature.
  • After state review the provider regained full reimbursement eligibility and avoided repayment by promptly correcting enrollment records and submitting supporting documentation.
