CHILDCARE PARENT PROGRAM AGREEMENT

Parties

Recitals

WHEREAS, Program Provider operates an organized childcare program offering supervised developmental, educational, and recreational activities for children; and

WHEREAS, Parent / Guardian is the legal guardian of the child named in this Agreement and desires to enroll the child in the Program Provider's Childcare Parent Program under the terms set forth herein; and

WHEREAS, the parties agree that the Program Provider will provide childcare services beginning on pursuant to the terms below.

Child Information

Scope of Work

Program Provider will provide childcare services including supervised play, educational activities, meals and snacks as scheduled, age-appropriate curriculum, routine diapering and restroom assistance, supervised rest periods, and transportation to and from program activities if explicitly provided below. Specific duties, hours, and days of attendance shall be set forth in the Program Schedule.

Payment Terms

Parent / Guardian shall pay Program Provider the fees described below in consideration for childcare services. Fees are due in advance according to the payment schedule selected.

All fees shall be payable to Program Provider at the address listed above unless otherwise agreed in writing. Late payments accrue interest or late charges as specified above and may result in suspension of services until balances are paid in full.

Term and Termination

This Agreement shall commence on and shall continue until unless earlier terminated in accordance with this section.

Either party may terminate this Agreement for any reason upon written notice to the other party delivered at least days prior to the effective termination date. Program Provider may terminate immediately for nonpayment, material breach, or behavior by the child or parent that jeopardizes safety. Termination shall not relieve Parent / Guardian of obligations to pay fees earned through the effective date of termination.

Confidentiality

Each party shall maintain in confidence any confidential information of the other party disclosed in connection with this Agreement and shall not use or disclose such information except as necessary to perform obligations under this Agreement or as required by law. Confidential information includes sensitive medical information, emergency contact information, and any other nonpublic information that a reasonable party would consider confidential. This obligation survives termination of the Agreement for a period of two years.

Parent / Guardian hereby authorizes the Program Provider to obtain emergency medical care for the child if Parent / Guardian cannot be reached. Parent / Guardian grants the following consents (check boxes as appropriate).

Policies

Parent / Guardian agrees to comply with Program Provider's policies regarding health and illness, drop-off and pick-up procedures, holiday closures, and behavioral expectations. Program Provider reserves the right to exclude a child exhibiting symptoms of communicable illness until cleared to return.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of without regard to its conflict of law principles. Any dispute arising under this Agreement shall be resolved in the appropriate state or federal courts located in that jurisdiction.

Entire Agreement

This Agreement, including any attachments and written policies incorporated by reference, constitutes the entire agreement between the parties regarding the subject matter hereof and supersedes all prior and contemporaneous agreements, representations, and understandings, whether written or oral. Any amendment or modification of this Agreement must be in writing and signed by both parties.

Severability & Miscellaneous

If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect. Headings are for convenience only and do not affect interpretation.
