What the Client Accommodation Form Is and when it’s used

A Client Accommodation Form documents a client’s request for a change to standard service delivery because of a disability, language need, mobility limitation, sensory impairment, or other accessibility requirement. Typical uses include requesting assistive devices, alternative communication formats, extended appointment times, or remote participation options. The form captures the requester’s contact details, description of the accommodation requested, any supporting documentation or medical verification required by policy, and preferred delivery or scheduling options. Organizations use the completed form to evaluate reasonable accommodations, track decisions, and document follow-up actions in a consistent, auditable way.

Who completes the form and who reviews it

Multiple roles interact with the Client Accommodation Form depending on setting and complexity.

Clear role definitions and a central intake point help ensure requests are acknowledged promptly and routed to the correct reviewer.

Core components every professional Client Accommodation Form should include

A complete form collects identity data, a detailed accommodation request, supporting evidence, decision tracking fields, signature, and routing metadata for compliance and recordkeeping.

Identity block

Fields for full legal name, date of birth or client ID, contact phone, email, and current mailing address to confirm identity and for follow-up communications.

Accommodation details

A structured area to state the requested accommodation, preferred options, frequency, and any assistive technology or physical adjustments needed.

Functional impact

Short narrative or checklist describing how the limitation affects participation, work, or access to services; helps reviewers assess reasonableness.

Supporting evidence

Upload fields or checkboxes for medical notes, vocational rehabilitation letters, evaluations, or third-party verification where policy requires documentation.

Decision record

Fields for reviewer name, determination (approved/denied/modified), implementation steps, reasonable timeframe, and appeal instructions if applicable.

Audit metadata

System fields for submission timestamp, reviewer actions, digital audit trail, and secure storage location to satisfy ESIGN, UETA, and retention obligations.

Security and compliance elements to include

Encryption in transit: TLS 1.2/1.3
Encryption at rest: AES-256
Audit trail: Timestamped events
HIPAA support: BAA required
Authentication: Multi-factor options
Standards: SOC 2 / ISO 27001

Digital signing and delivery considerations

Choose submission and signing options that meet legal and privacy requirements for your setting.

  • Document formats: PDF, DOCX supported
  • Auth options: Email, SMS, KBA
  • Integrations: CRM and cloud storage

Verify the eSignature provider supports required compliance features (HIPAA, ESIGN/UETA, audit trails) and integrates with your record systems before enabling eSubmission.

How to configure an online accommodation workflow

Field Configuration
Intake form Required fields: identity, accommodation, evidence upload
Routing Auto-send to ADA coordinator or assigned reviewer

  • Acknowledge receipt: System auto-sends confirmation to requester.
  • Review and decision: Coordinator assesses request, consults necessary parties, documents outcome.

Key risks and compliance concerns

Mismatched identity: May invalidate the request
Insufficient evidence: Leads to delays or denial
Unauthorized disclosure: HIPAA or privacy breach risk
Missed deadlines: Regulatory complaint potential
Improper notarization: Legal challenge to authenticity
Poor audit trail: Weakens defensibility

Common mistakes that slow processing

  • Submitting incomplete requester contact information, which prevents timely clarifying questions and delays implementation.
  • Using vague accommodation descriptions such as 'assistive help' instead of specifying the exact device, format, or interpreter requested.
  • Failing to attach required supporting documentation when the policy mandates verification, causing requests to be returned as incomplete.
  • Storing completed forms in unsecured locations or personal drives instead of encrypted, access-controlled repositories required by policy.

Practical tips to speed review and preserve compliance

Apply consistent intake, clear documentation standards, and secure e-signature workflows to reduce friction and support defensible decisions.

Standardize intake fields
Design the form with required fields for identity, precise accommodation request, functional impact, and evidence uploads. Mandatory fields prevent common omissions and reduce back-and-forth communications with requesters.
Use role-based routing
Automate assignment to ADA coordinators or program managers and set notification SLAs. Role-based workflows prevent bottlenecks and provide clear accountability for decisions and implementation steps.
Enable secure eSubmission
Accept electronic signatures that comply with ESIGN and state laws, secure uploads with TLS/AES encryption, and require multi-factor authentication for sensitive records when appropriate to reduce privacy risk.
Maintain an audit trail
Record timestamps, reviewer actions, and communications. A detailed audit trail supports appeals, regulatory reviews, and internal quality assurance.
