What a Client Account Info Request Is and when it’s used

A Client Account Info Request is a standardized form used to collect core account details from a client or counterparty, including legal name, tax identifiers, contact information, and account setup preferences. Organizations use this document to verify identity, ensure correct tax reporting, and configure billing or service access. The form supports recordkeeping and audit requirements and can be delivered on paper or electronically. When completed accurately, it reduces payment delays, prevents backup withholding triggers, and provides a clear record for compliance and customer service teams.

Why collecting client account information matters

Accurate account data ensures correct tax reporting, prevents payment interruptions, and creates an auditable trail for compliance. It also reduces onboarding time and supports automated workflows when integrated with systems of record.

Why collecting client account information matters

Who typically sends and completes this request

Completed requests form part of the client record used for billing, tax reporting, and ongoing account maintenance.

  • Accounts payable teams verifying vendor TINs and payment details for 1099 reporting.
  • Client onboarding teams collecting legal entity and contact information to set up accounts.
  • Compliance or legal teams confirming identity and consent for electronic records.

Who should sign and who approves

Authorized Signer

An officer, owner, or delegated employee with authority to bind the client must sign the request. For companies, use the individual listed in corporate resolution or delegated authority records to avoid disputes and ensure that tax and banking information is legally attributable.

Requesting Officer

Internal approver is typically a finance manager or vendor manager who validates data, confirms supporting documents, and approves setup. This control reduces erroneous payments and ensures separation of duties in onboarding workflows.

Core sections to include in a professional request

A complete Client Account Info Request groups related details so reviewers can verify identity, tax status, and payment preferences quickly. Organize sections to match verification steps and required attachments.

Client Identity

Legal name, DBA (if any), entity type (individual, LLC, corporation), and government-issued identifier to match tax and bank records.

Tax Information

Taxpayer Identification Number (TIN/EIN/SSN) and taxpayer classification (individual, sole proprietor, corporation) used for IRS reporting and backup withholding determinations.

Contact details

Primary contact name, phone, email, and physical address for notices, payment remittance, and service-related communications.

Payment instructions

Preferred payment method (ACH, check, wire), account and routing details where applicable, and remittance contact information.

Supporting documents

Copies of W-9 or equivalent tax form, bank authorization, and any identity documents required for higher-risk accounts.

Signature and date

Signature block with printed name, title, date, and authority statement affirming accuracy of the information provided.

Typical online workflow settings for electronic completion

Configure fields and authentication to match your verification and audit requirements.

Field Configuration
Required fields Mark name, TIN, and signature as mandatory.
Authentication Use email plus optional SMS code or KBA for higher assurance.
Attachments Allow PDF, DOCX uploads and require W-9 or bank form when applicable.

Typical end-to-end process when you send the request electronically

Electronic workflows reduce turnaround by guiding signers and capturing audit data automatically.

  • Upload and tag: Upload the request and place required fields for the signer.
  • Send or share link: Deliver via secure email invite or a protected signing link.
  • Signer completes: Signer authenticates, fills fields, attaches documents, and signs.
  • Record stored: Signed copy and audit trail are stored for retention and reporting.

Technical and security considerations for eSubmission

Align platform settings with your compliance policies (HIPAA, financial rules) and retention schedules before production use.

  • Authentication: Email, SMS, KBA, or advanced signer verification options.
  • Encryption: TLS in transit and AES-256 at rest required.
  • Integrations: CRM, ERP, and cloud storage integrations streamline onboarding.

Timing considerations and when to request updated account info

Request account info at onboarding, before the first payment, and whenever tax or payment details change to avoid reporting problems and backup withholding.

Initial onboarding:

Collect before first invoice to ensure correct payments and tax categorization.

Before tax reporting:

Verify TINs and classifications ahead of year-end information return deadlines.

Upon ownership changes:

Request updated documents when control or structure changes occur.

Periodic review:

Revalidate high-value vendors annually or per your risk policy.

Upon request:

Clients may be asked to provide updated info at any time.

Common preparation mistakes to avoid

  • Submitting incomplete TINs or mismatched legal names, which can trigger backup withholding or rejected payments and require time-consuming corrections.
  • Accepting unsigned or undated forms, creating audit gaps and raising questions about signer authority during reviews.
  • Failing to require or review supporting documents like W-9s and bank authorizations, which increases fraud and payment failure risk.
  • Using weak signer authentication for sensitive accounts, which can complicate regulatory response and dispute resolution.

Consequences of incorrect or missing client account information

Backup withholding: 24% withholding may apply for missing/incorrect TINs
IRS penalties: Penalties for incorrect information returns can apply
Payment delays: Payments held until verification completes
Compliance risk: Regulatory scrutiny for inadequate controls
Contract disputes: Signature or authority issues can void actions
Fraud exposure: Insufficient verification increases fraud risk
