What the Client Acknowledgment and Direction Form Is

The Client Acknowledgment and Direction Form documents a client's receipt of specified information and records their specific instructions or authorization to act. It combines an acknowledgment (that the client received particular disclosures or documents) with a directional clause (authorizing a party to take or refrain from an action). Organizations use this form to create a clear, auditable record of client consent, routing instructions, or service directions in both paper and electronic workflows.

Why this form matters for compliance and clarity

The form reduces ambiguity about client intent, helps meet recordkeeping and consent obligations, and creates an evidentiary trail if disputes arise. Properly completed acknowledgments support compliance with ESIGN (15 U.S.C. ch. 96) and state UETA frameworks while documenting instructions that guide internal processing.
