What the Client App Signatures Form Is

The Client App Signatures Form is a structured document used to collect legally binding signatures and attestations from clients for application-level access, consent, or service enrollment. It records signer identity, scope of permissions, effective dates, and any required acknowledgements. In the United States this form is typically executed under ESIGN (15 U.S.C. §7001) or a state UETA framework and may include fields for authentication, notarization, or auxiliary disclosures where required by industry rules or organizational policy.

Why a Professional Signatures Form Matters

A clear, correctly completed Client App Signatures Form documents intent, consent, and attribution—key elements for enforceability under ESIGN and UETA. It reduces disputes, streamlines onboarding, and supports auditability and compliance.
