What the Client Application & EFT Form Is and when it’s used

The Client Application & EFT Form combines client onboarding information with electronic funds transfer (EFT/ACH) authorization so businesses can collect identity, tax, and bank account details and obtain consent to debit or credit an account. Typical content includes full legal name, business or individual TIN, mailing and remittance addresses, routing and account numbers, authorization language for recurring or one-time transfers, and disclosures required for consumer-facing transactions. It is used to set up payment arrangements, verify payer identity for tax reporting, and document consent in a way that supports electronic execution and long-term record retention under U.S. law.

Why a clear Client Application & EFT Form reduces billing friction

A well-structured form centralizes the data needed to start billing, avoid rejected ACH transactions, and meet tax and regulatory reporting obligations while documenting payer consent.
