What the Client Case Setup Document Is

The Client Case Setup Document is a standardized intake and record template used to capture essential facts, parties, permissions, and workflow instructions when opening a new client matter or project. It centralizes contact data, scope of services, fee terms, required supporting documents, signer authority, confidentiality preferences, and routing instructions for approvals or signatures. Designed for legal, professional, and administrative teams, the document creates an auditable starting point for downstream tasks, compliance checks, billing, and storage. Accurate completion reduces rework and supports retention and regulatory obligations.

Why a Standardized Setup Document Matters

A Client Case Setup Document clarifies responsibilities, captures necessary legal and administrative data, and standardizes routing for signatures and approvals. That consistency lowers onboarding delays, reduces compliance risk, and preserves an audit trail for later review.

Why a Standardized Setup Document Matters

Who Prepares and Relies on This Document

Common users who prepare or rely on this document include in-house counsel, case managers, intake coordinators, and client-facing account managers.

  • Legal teams ensuring scope, authority, and confidentiality terms are recorded before service begins.
  • Finance or billing staff tracking fee terms, invoicing schedules, and billing contacts.
  • Account managers and operations staff coordinating document routing, signatures, and required supporting files.

Across teams, the form reduces duplicated intake steps and creates a single source of truth for managing a client matter from opening through closure.

Core Sections to Include in a Professional Setup Document

A professional Client Case Setup Document groups intake data into logical sections to speed review, standardize approvals, and ensure required legal, billing, and operational details are captured up front.

Client Details

Record full legal name, primary contact, mailing and service addresses, tax identification where applicable, and preferred communication channels to avoid delays and mismatches during onboarding.

Scope of Work

Describe services or matter scope with clear start and end points, deliverables, milestones, and any excluded tasks to prevent scope creep and billing disputes thereby.

Fee Terms

Specify fee structure, payment methods, invoicing cycles, retainers or deposits required, expenses reimbursement, and backup withholding requirements when taxpayer ID is incomplete or late fees apply.

Signatory Authority

Identify individuals authorized to sign, include job title and capacity (e.g., agent, trustee, officer), and confirm any delegated authority or required corporate resolutions and signature specimen where appropriate.

Supporting Documents

List required attachments such as IDs, W-9s, incorporation documents, insurance certificates, POA, or prior agreements, with instructions on acceptable formats and where to upload electronically.

Common Preparation Mistakes to Avoid

  • Incomplete or inconsistent client names and TINs cause payment holds, backup withholding, and require reissuance of corrected tax forms, delaying revenue recognition and increasing administrative overhead.
  • Missing signer authority records or unsigned corporate resolutions produce invalid signatures, force rescission or re-signing, and can expose the organization to contractual disputes or non-enforceability.
  • Uploading poor-quality scans, wrong file types, or unsearchable images prevents verification, increases processing times, and may lead to requests for physical documents or notarized copies.
  • Not confirming HIPAA or data-sharing requirements before sending forms with health information can breach regulations and require execution of a Business Associate Agreement.

Penalties and Legal Risks from Incorrect Setup

1099 Filing Penalties: $60–$330 per form; $660+ intentional disregard.
W-2 Penalties: Mirror 1099 penalties under IRC §6721.
I-9 Violations: $281–$2,789 per violation (8 CFR §274a.2).
HIPAA Noncompliance: Fines and corrective action; 45 CFR §164.530(j).
Contract Invalidity: Incorrect signatory authority may void agreement.
Data Breach Liability: Encryption and audit gaps increase legal risk and exposure.
