What the Client Change of Race Form Is

The Client Change of Race Form is a standardized administrative document used to record and update an individual's self-identified race or ethnicity in client, patient, or student records. Organizations use it to correct legacy data, align demographic records across systems, and document the client's stated identity. The form generally captures prior and updated race selections, the effective date, a reason for the change, any supporting evidence, and required signatures from the client and authorized staff. Proper completion creates an auditable record for reporting and compliance while protecting privacy.

Why a Formal Change Form Matters

Updating race information preserves client autonomy, improves data accuracy for compliance and reporting, and reduces mismatches across systems. A formal Client Change of Race Form documents intent, captures necessary approvals, and creates an audit trail that supports nondiscrimination obligations and statistical integrity.
