What the Client Character Form Is and When It’s Used

The Client Character Form is a standardized questionnaire used to collect background, conduct, references, and suitability information about an individual or organization being considered as a client, vendor, or party to a professional engagement. Typical uses include intake for legal representation, background checks for fiduciary roles, placement of clients in sensitive programs, and onboarding where character, conflicts, or reputation are material to the relationship. The form consolidates identity details, contact data, employment and education history, references, disclosures of prior disciplinary items, and attestations to be retained in the client file.

Why a Formal Client Character Form Matters

A consistent Client Character Form creates an auditable record of the information considered when accepting or continuing a client relationship, reduces onboarding errors, and supports regulatory and professional diligence obligations such as conflict screening and ethics review.

Why a Formal Client Character Form Matters

Who Typically Completes and Reviews This Form

Use role-based routing so the right reviewer sees attestations and supporting documents without exposing unrelated personal data.

  • Intake and Compliance Officers — run identity checks, conflict screening, and initial risk scoring during onboarding.
  • Legal and Ethics Counsel — review assertions about previous disciplinary actions or legal proceedings for conflicts and professional responsibility.
  • HR and Credentialing — verify employment history, professional licenses, and references where suitability is required.

  • Audit & Store: System captures timestamps, IPs, and stores the audit trail.

Sharing and eSubmission Channels for the Client Character Form

Ensure the chosen channel supports an unalterable audit trail and the retention policy required by your compliance rules.

  • Email Link: Works for low-risk cases; use click-through consent and optional SMS code.
