The Client Comment Issues Form is a standardized document used to capture, categorize, and track client feedback, complaints, and issue reports across project, service, and support lifecycles. • Using informal severity labels without documented criteria leads to inconsistent triage and missed escalations, which can expose the organization to SLA breaches and reputational harm.

Best Practices to Keep Records Accurate, Compliant, and Actionable

Follow these best practices to ensure accurate, defensible Client Comment Issues Forms that support timely resolution, compliance reviews, and reliable reporting across teams.

Use standardized severity criteria and definitions
Define clear severity levels with examples and measurable impact thresholds. Train intake staff to apply criteria consistently and require a brief rationale for each assigned level to support prioritization and later disputes or audits.
Capture verifiable submitter identity and consent
Record the submitter's legal name, contact method, and any authority to act on behalf of an organization. For electronic submissions, capture consent to transact electronically and retain evidence of access and authentication per ESIGN and UETA.
Attach corroborating evidence and logs
Include screenshots, server logs, transaction IDs, and related correspondence to corroborate the reported issue. Use consistent filenames and reference attachments in the description so investigators and auditors can quickly match evidence to entries.
Review and close with acknowledgement
Before closing, confirm the client or submitter acknowledges the resolution in writing or electronically, document any remaining follow-up actions, and update the record. Archiving should preserve tamper-evident evidence and meet retention policies applicable to the content.
