What the Client Comprehensive Assessment Form Is

The Client Comprehensive Assessment Form is a structured intake document used to collect demographic, clinical, social, financial, and consent-related information about a client. It centralizes history, presenting issues, risk factors, supports, and service needs so teams can triage, plan interventions, document informed consent, and meet regulatory or funding requirements.

Why this form matters for accurate client care

A complete, standardized assessment reduces practice variation, supports defensible clinical decisions, documents consent and eligibility, and preserves a single source of truth for audits, billing, and interagency coordination.
