What the Client Consent and Authorization Form Is

A Client Consent and Authorization Form documents a client’s informed agreement to specific actions, disclosures, or services and grants the provider defined authority to act on the client’s behalf where indicated. It records identity, scope of consent, effective dates, and any limits on disclosure or action. In the United States this kind of form is enforceable as an electronic or paper record when it meets ESIGN and applicable state electronic transaction rules. Use the form to clarify permissions, create an audit trail, and reduce downstream disputes over authority or information sharing.
