What the Client Data IPLV Form Is and when it’s used

The Client Data IPLV Form is a standardized client intake and data verification template used to collect identifying information, contact details, consent statements, and verification artifacts for onboarding, compliance checks, or account updates. Practical tips for accurate and efficient form completion

Adopt these practices to reduce errors and limit the need for rework when collecting client data.

Standardize formats
Require precise formats for dates, state abbreviations, and identification numbers. Enforce masks and validation to prevent transposition errors and to speed automated processing.
Pre-validate required docs
Ask clients to attach scanned ID and proof of address at submission. Early validation reduces repeat requests and accelerates identity verification workflows.
Use conditional fields
Show only relevant fields based on prior answers to shorten the form, reduce user confusion, and minimize accidental disclosure of unnecessary data.
Capture consent explicitly
Include clear electronic-consent language that meets ESIGN consumer disclosure requirements and records the method of consent and timestamp for future audits.
