What the Client Dave Form Is and when it’s used

The Client Dave Form is a standardized client intake and authorization document used to collect identifying information, contact details, scope and terms of engagement, and signature authority from a client named Dave or similarly identified contacts. It functions as both a data collection form and a basic agreement that confirms consent, service parameters, billing instructions, and preferred communications. Organizations use this form during onboarding, contract initiation, or when updating client records; the completed form creates a documented record that supports billing, compliance, and downstream processing.

Why a clear Client Dave Form matters

A correctly completed Client Dave Form reduces onboarding delays, clarifies responsibilities, and generates a retrievable record for audits and customer service while supporting electronic execution standards under ESIGN and UETA.

Company — Record Owner

The internal employee (for example, account manager or contracting officer) who completes intake, verifies identity, and stores the form in the official records system. This person is responsible for retention and follow-up.
