What the Client Enquiry Questionnaire Is

A Client Enquiry Questionnaire is a structured form used to collect essential client information during onboarding or before a project starts. It combines contact and identification details, project or service scope, budget and timing expectations, compliance disclosures, and required attachments into a single record. The questionnaire standardizes data capture, reduces back-and-forth email threads, and creates an auditable record that can be retained as part of client files. It is suitable for manual completion, digital completion, and electronic signing where parties consent to e-records.

Why a Standard Questionnaire Matters

A consistent Client Enquiry Questionnaire reduces onboarding time, improves data quality, and creates a single authoritative source for client expectations. When executed electronically with proper consent and audit trails, the questionnaire meets U.S. e-signature standards under ESIGN and state UETA laws for most commercial transactions.

Why a Standard Questionnaire Matters

Who Prepares and Who Completes the Questionnaire

Different teams may issue or complete the questionnaire depending on the organization and use case.

  • Sales or business development teams send the questionnaire to prospective clients to capture scope and qualification details before contract drafting.
  • Client services and project managers use the questionnaire to gather technical requirements, timelines, and stakeholder contact information for planning.
  • Clients or their authorized representatives complete the questionnaire and supply supporting documents, identification, and consent where required.

Use role-based routing so the issuing team, reviewers, and approvers receive the questionnaire version they need without manual handoffs.

Primary Signers and Administrators

Client Representative

The person authorized to provide information and consent on behalf of the client entity. This individual should have the legal authority to bind the organization and must match the name on government identification or corporate records.

Internal Approver

The company employee responsible for reviewing questionnaire responses, verifying supporting documents, and triggering downstream actions such as contract generation, credit checks, or project intake.

Core Sections Every Professional Questionnaire Should Include

A well-designed Client Enquiry Questionnaire groups related items and flags required fields to prevent incomplete submissions.

Client Identity

Full legal name, entity type, and any DBA information. Accurate legal names are required for contracts and tax reporting to avoid later mismatches.

Contact Details

Primary email, direct phone, postal address, and emergency contact. Provide business and billing addresses separately to prevent delivery errors.

Project Scope

Concise description of services requested, deliverables, and acceptance criteria. Clear scope reduces scope-creep disputes and clarifies expectations.

Budget & Timeline

Estimated budget, billing preferences, and target dates. This helps internal approval routing and resource planning.

Compliance & Consents

Data-processing, HIPAA or FERPA disclosures where applicable, and consumer-facing e-consent language to meet ESIGN consumer disclosure rules.

Supporting Attachments

Fields for uploading IDs, invoices, contracts, or proposals. Specify accepted file types and max size to avoid upload failures.

  • Email to team: Direct delivery to onboarding or sales inbox for immediate review

  • Failing to collect explicit e-consent when the form triggers consumer-facing disclosures required by ESIGN.
