What the Client Fact-find Form Is and When It’s Used

A Client Fact-find Form is a structured intake document used to collect standardized personal, contact, identification, financial, and consent information from a client at the start of an engagement. It centralizes data required for onboarding, risk assessment, compliance screening, and document generation. Typical uses include financial advising, legal intake, real estate client setup, healthcare administrative intake, and vendor onboarding. Properly completed fact-find forms reduce follow-up questions, speed downstream processes, and establish a verifiable record of the client’s declared details and consents for future reference.

Why a Complete Client Fact-find Form Matters

A thorough fact-find minimizes risk, enables accurate disclosures and regulatory compliance, and speeds service delivery by providing a single source of truth for client data.

Why a Complete Client Fact-find Form Matters

Who Typically Prepares and Completes the Form

The completed form becomes part of the client record and may be used to populate agreements, filings, or regulatory disclosures.

  • Financial advisors and wealth managers collecting income, assets, and investment objectives for suitability analysis.
  • Attorneys and legal intake teams capturing identity, matter details, and conflict-of-interest disclosures.
  • Healthcare administrative staff collecting demographic and consent data, often combined with HIPAA authorizations.

Core Sections a Professional Client Fact-find Form Should Include

A well-constructed fact-find groups related information, uses clear labels, and includes consent and verification elements so data can be relied on for contracts and compliance checks.

Client Identity

Full legal name, any DBAs, government ID numbers, and date of birth to verify identity and match background checks or tax reporting.

Contact Details

Street address, mailing address, primary phone, secondary phone, and email address for service, legal notices, and emergency contact use.

Social Security number or TIN, taxpayer classification, and withholding instructions where relevant for tax reporting and backup withholding decisions.

Financial Snapshot

Summary of assets, income, liabilities, and account preferences used to assess suitability, fees, or billing arrangements.

Consents & Disclosures

Consent to electronic records, privacy disclosures (HIPAA/FCRA as applicable), and acknowledgements required for consumer-facing transactions.
