What the Client Fact Finder Form Is and when it’s used

The Client Fact Finder Form is a structured intake document used to gather a client’s identifying, contact, financial, and engagement-related information during onboarding. It typically collects legal name, contact details, tax identifiers, employment and income data, risk tolerance, objectives, and relevant documents. Firms use the form to establish records for compliance, suitability analysis, billing, and ongoing communications. The form may be delivered as a fillable PDF, web form, or an electronically signed record; signNow is a secure, compliant eSignature solution commonly used across U.S. industries to capture signed fact finders.

Why a well-completed Client Fact Finder matters

A complete, accurate Client Fact Finder reduces onboarding friction, supports regulatory recordkeeping, enables tailored advice, and documents consent and disclosures. It also creates a defensible record of client facts and preferences used for service delivery and dispute resolution.

Essential components of a professional Client Fact Finder

A complete form balances clarity, required fields, and conditional logic to collect only relevant data while preserving an audit trail.

Identifying data

Fields for full legal name, DOB, SSN/TIN, and government ID numbers. These items support identity verification and tax reporting and should match official documents.

Contact and address

Street address, mailing address, phone, and preferred email. Use two-letter state abbreviations and full ZIP codes to avoid delivery or legal-service problems.

Financial snapshot

Income ranges, asset classes, liabilities, and liquidity notes. This section drives suitability assessments and ongoing monitoring for financial services and advisory relationships.

Engagement details

Scope of services, fee arrangement, billing contact, and effective date. Clear engagement terms reduce downstream disputes and clarify responsibilities.

Authorizations

Consent for information sharing, third-party checks, and electronic delivery. Include explicit consumer disclosures where required by ESIGN for consumer-facing records.

Supporting uploads

Placeholders for ID images, proof of address, tax documents, and signed disclosures to create a consolidated record for compliance and auditing.

Common mistakes when preparing the Client Fact Finder

  • Mismatched names between the form and government ID, which can delay identity verification and tax reporting.
