What the Client Feedback Touch Ups document captures

The Client Feedback Touch Ups is a standardized form used to record, prioritize, and authorize small changes requested by a client to a deliverable or asset. It documents the specific edits requested, attaches or references affected files, captures screenshots or notes, assigns priority and estimated effort, and collects the approver's sign-off. The form supports version control and a clear audit trail so teams can reconcile requests with final deliverables, reduce rework, and maintain a record for invoicing or change-order purposes.

Why a formal touch-ups record matters

A concise feedback touch-ups form reduces ambiguity, speeds approvals, and creates an auditable record for scope, timing, and responsibility. When executed correctly and, if needed, e-signed under ESIGN or state law, it helps prevent disputes and supports accurate billing or change orders.

Why a formal touch-ups record matters

Who typically completes and signs this form

The Client Feedback Touch Ups form is used by internal teams and external clients to document requested refinements, route approvals, and confirm acceptance before work proceeds.

  • Project Managers — Centralize client requests, assign work, and track scope changes across team members and milestones.
  • Creative/Production Leads — Translate requested edits into actionable tasks, estimate effort, and confirm feasibility before scheduling.
  • Clients or Client Representatives — Provide precise change descriptions, attachments, and final sign-off to authorize implementation.

Use clear role designations on the form so each item has an accountable requester, reviewer, and approver to avoid duplication and missed requests.

Step-by-step: complete and close a touch-up request

Follow these four core steps to capture, validate, implement, and finalize client touch-ups with clear accountability and a record for audit and billing.

  • 01
Prepare the form: Open template, enter project ID and client details.
  • 02
Record the feedback: Document change, attach files, set priority and ETA.
  • 03
Authorize work: Obtain approver signature or validated e-signature.
  • 04
Implement and confirm: Make edits, attach revised file, collect final acceptance.

Configure an online workflow for touch-ups

Set up a repeatable template and routing so requests are automatically assigned, authenticated, and tracked in your system of record.

Field Configuration
Template Name Client Feedback Touch Ups
Recipient Roles Requester | Reviewer | Approver
Authentication Email link by default; SMS code optional
Notifications Email on assignment and on completion

How electronic completion and sharing typically work

Choose a platform that supports PDF/DOCX uploads, role-based fields, and an audit trail so signed touch-ups are reproducible and enforceable.

  • Integrations: Salesforce, NetSuite, Google Workspace
  • File formats: PDF, DOCX, PNG, JPG
  • Security: TLS in transit; AES-256 at rest

Typical electronic sign-and-return flow

A simple electronic workflow makes it easy for clients to review and approve touch-ups without installing software or managing multiple file versions.

  • Upload: Sender uploads form and attachments to the signing platform.
  • Place fields: Add signature, date, and approver fields in the template.
  • Send to signer: Signer receives secure link by email or SMS.
  • Audit trail: Platform records timestamp, IP, and completion certificate.

Essential elements of a professional touch-ups form

A complete Client Feedback Touch Ups form balances clarity, evidence, and approval workflow so changes are implemented correctly and traceably.

Clear change description

Concise description plus bulletized specifics prevents interpretation errors and reduces back-and-forth clarifications during implementation.

Annotated references

Screenshots or marked-up files show exactly where edits are required, eliminating guesswork for designers and developers.

Version control

Record original and revised file names and versions to prevent overwriting and preserve auditability for billing or disputes.

Assigned owner

A named owner coordinates the work, tracks status, and confirms completion with the client to close the loop.

Approval fields

Designated signers and date fields capture explicit authorization and show who approved scope or budget changes.

Audit metadata

Timestamps, IP address, and change log entries provide proof of consent and sequence of events.

Required data points to capture on every touch-up

Client Name: Full legal name
Project Code: Internal identifier
Change Summary: One-sentence description
Affected Files: Filename and version
Requested By: Name and role
Approval Status: Pending, Approved, Rejected

Risks of incomplete or inaccurate touch-ups records

Delayed delivery: Missed deadlines
Scope disputes: Unclear requests
Rework costs: Additional billable hours
Lost versions: Overwritten assets
PHI exposure: HIPAA violations risk
Billing delays: Invoice disputes

Common mistakes to avoid when preparing touch-ups

  • Vague descriptions — failing to reference exact file versions leads to incorrect edits and unnecessary rework.
  • Missing attachments — omitting screenshots or source files forces guesswork and often requires follow-up clarification.
  • Multiple concurrent approvers — not defining a single approver causes conflicting directions and stalled progress.
  • Incorrect metadata — wrong project IDs or client names disconnect the request from billing and contract records.

Practical tips for accurate and efficient completion

Adopt small process improvements to reduce friction and ensure every touch-up is actionable, auditable, and aligned with contract terms.

Use a single template for all clients
Standardized templates reduce ambiguity. Include defined fields for project ID, file reference, change description, priority, estimated effort, and approver signature so nothing is omitted.
Require one approver per change
Designate a single authorized approver to prevent conflicting approvals and to make the signatory attributable for billing or legal purposes.
Attach annotated visuals
Screenshots with marked-up changes are more reliable than text alone and cut back-and-forth clarification by showing exactly where edits belong.
Enable digital signatures and audit logs
Store a certificate of completion with each signed form showing timestamp, signer identity, and IP address to support attribution and chain-of-custody.

Real-world examples of using the touch-ups form

Two practical scenarios show how a clear touch-ups process prevents rework and clarifies billing and approvals across teams.

Marketing Agency Project

A client requests two visual revisions and a copy change to a landing page in one request.

  • The agency attaches screenshots, marks elements, and estimates one business day.
  • Final acceptance is e-signed; the signed record ties the edits to a single invoice line, preventing disputed charges and speeding payment.

Healthcare Patient Materials

A hospital updates patient consent language and requests small formatting edits across brochures.

  • PHI is not included; attachments are encrypted.
  • The compliance lead reviews and signs electronically under a HIPAA-compliant workflow, and copies are retained for six years per HIPAA rules.

Timing expectations and standard deadlines

Define deadlines on each request so teams can triage work and clients know when to expect delivery; align internal SLAs with client priorities.

Immediate Acknowledgement:

Respond within 24 business hours to confirm receipt

Preliminary Response:

Provide feasibility and ETA within 48–72 hours

Revision Window:

Standard minor edits completed within 1–5 business days

Final Approval Deadline:

Client to approve within agreed window to avoid delays

Implementation Date:

Enter the date when edits are published or delivered
