What the Client Field Screen Questionnaire Is

Client Field Screen Questionnaire is a structured intake form used to capture project-specific details, client contact data, on-site conditions, and authorization information before field work begins. It organizes fields for client identification, location, scope of services, access instructions, safety notes, and consent statements to support operational planning and compliance. Used by project managers, technicians, and inspectors, the questionnaire reduces follow-up queries and documents client approvals. When completed accurately it becomes part of the project record and supports billing, risk assessments, and any required regulatory documentation.

Why Standardize Intake with a Questionnaire

Use the Client Field Screen Questionnaire to standardize client intake, reduce onsite delays, and create an auditable record of client approvals and site conditions. Standardization lowers risk, supports accurate billing, and simplifies regulatory compliance where documentation is required.

Why Standardize Intake with a Questionnaire

Who Typically Completes or Reviews This Questionnaire

On-site technicians, project managers, and client service representatives typically complete or review the Client Field Screen Questionnaire to confirm scope and access.

  • Field technicians collect site details, immediate hazards, and access instructions before starting work.
  • Project managers use responses for resource planning, scheduling, and scope verification.
  • Client representatives confirm contact details, authorization, and billing information to prevent discrepancies.

Accurate completion improves project outcomes, reduces rework, and creates a defensible record for invoicing and disputes.

Core Sections to Include in a Professional Questionnaire

A professional questionnaire groups client identification, site details, safety notes, authorization, photo evidence fields, and signature blocks to support clear, actionable field operations and recordkeeping.

Client Identification

Collect full legal name, organization (if applicable), email, phone number, and mailing address; instruct to enter names exactly as on government ID to avoid mismatches during verification or invoicing.

Site Details

Specify site address, unit or suite, GPS coordinates where relevant, best access points, parking restrictions, security contacts, and expected onsite hazards so crews can prepare and carry necessary equipment.

Scope of Work

Describe tasks to be performed, materials to be used, estimated duration, key milestones, and acceptance criteria. Clear scope minimizes disputes and supports accurate time and materials billing.

Safety & Access

Record required personal protective equipment, site safety protocols, confined-space alerts, permit requirements, and point-of-contact for emergency access so teams meet regulatory and customer safety obligations.

Photo & Evidence

Provide fields for photographs, measurements, serial numbers, and optional attachments. Timestamped images and descriptions support post-visit reports and reduce later disputes over condition or installations.

Authorizations & Signatures

Include signature block, printed name, role, date, and consent statements for access or work authorization. Specify whether initials suffice or full signatures are required for legal validity.

Step-by-Step: Completing the Questionnaire

Follow these steps to complete the Client Field Screen Questionnaire accurately and efficiently in one session when possible.

  • 01
Prepare: Gather IDs, permits, and prior reports before starting the form.
  • 02
Complete Fields: Enter client and site data using exact formats.
  • 03

Common Preparation Mistakes to Avoid

  • Incomplete contact information forces follow-up calls, delays site access, and can lead to billing mismatches that slow project closeout and increase administrative overhead.
  • Vague scope descriptions omit exclusions and deliverables, creating scope creep and disputes about whether work was authorized or falls under warranty obligations.
  • Missing or poor-quality photos prevent accurate post-visit assessments, reducing evidence quality for claims, warranty work, or insurance adjustments.
  • Incorrect signature types or unsigned consent fields can invalidate authorization, cause payment disputes, and may require costly reauthorization or onsite revisits.

Consequences of Incomplete or Incorrect Questionnaires

Invalid Authorization: Work may be unauthorized; liability exposure.
Billing Disputes: Invoices delayed or challenged.
Regulatory Breach: HIPAA violations if PHI mishandled.
Contract Delays: Project start postponed.
Reauthorization Costs: Extra site visits and fees.
Auditor Findings: Retention lapses trigger penalties.
