What the Client Fraud Questionnaire Is and When It Applies

A Client Fraud Questionnaire is a structured form used to collect factual information when potential fraud, misrepresentation, or suspicious activity is reported or detected in a client relationship. It captures identifying information about the client and reporter, a timeline of relevant events, transaction details, supporting evidence, and a signed declaration confirming the accuracy of responses. Organizations use the questionnaire to support internal investigations, regulatory reporting, insurance claims, or law enforcement referrals while preserving an auditable record of the intake and review process.

Why a Standardized Questionnaire Matters for Fraud Response

A consistent Client Fraud Questionnaire helps ensure accurate fact collection, preserves evidence, and supports defensible decision-making in investigations. It reduces information gaps and creates an auditable record that can be used for internal controls, regulatory inquiries, or legal proceedings.

Why a Standardized Questionnaire Matters for Fraud Response

Who Completes and Reviews the Questionnaire

The questionnaire is completed by the employee, investigator, or third party who first documents the suspected fraud; it is then routed to the appropriate review team.

  • Internal compliance or fraud team members who conduct preliminary intake and triage of reported incidents.
  • Account managers or client service representatives who collect client-facing details and initial supporting documents.
  • Legal, risk, or external counsel for review when legal exposure, regulatory reporting, or litigation is possible.

Retain copies of completed questionnaires in both the investigative file and the organization’s records system for auditability and future reference.

Core Sections to Include in a Professional Client Fraud Questionnaire

A complete questionnaire organizes intake data to support investigation, evidence preservation, and downstream reporting; structure improves consistency and reduces follow-up.

Reporter Identity

Collect the reporter’s full legal name, job title, department, contact details, and relationship to the client to enable follow-up and preserve chain-of-custody for statements.

Client Identification

Enter the client’s legal name, account number, primary contact, and any alternate identifiers (TIN, DOB) used internally to ensure correct record linkage.

Incident Timeline

Capture dates and times of discovery, relevant transaction dates, and a concise chronological narrative of events to support forensic and regulatory review.

Transaction Details

List transaction identifiers, amounts, payment methods, origins/destinations, and any unusual account activity that could indicate diversion or falsification.

Supporting Evidence

Describe and attach documents, screenshots, emails, wire confirmations, and any vendor or witness statements that substantiate the reported facts.

Declaration & Signature

Include a signer certification block with printed name, role, date, and signature (electronic or handwritten) to record intent and attribution.

Step-by-Step: Completing and Routing the Questionnaire

Follow this sequence to gather facts, validate evidence, and escalate appropriately while maintaining an audit trail.

  • 01
Step 1: Gather incident facts and supporting documents before starting the form.
  • 02
Step 2: Complete all identification and timeline fields with exact dates and identifiers.
  • 03
Step 3: Attach or upload supporting evidence and summarize key items in the evidence section.
  • 04
Step 4: Sign the declaration and route the completed questionnaire to compliance and legal.

Where Completed Questionnaires Should Be Sent

Use the following routing options to ensure the questionnaire reaches the correct internal or external recipients for review and action.

  • Internal Compliance: Primary destination for intake, triage, and assignment to investigators.
  • Legal Review: Send when potential litigation, regulatory reporting, or preservation letters are required.
  • Risk/Finance: Forward transaction-related items for reconciliation and payment hold as needed.
  • Law Enforcement: If criminal activity is suspected, prepare a referral packet with redacted supporting evidence.

Configuring an Online Questionnaire Workflow

Set up digital intake with clear fields, required attachments, signer authentication, and routing to streamline processing.

Field Configuration
Required Fields Mark reporter, client, incident date, and declaration as required.
Attachments Allow PDF, JPG, PNG uploads; enforce a maximum file size per attachment.
Authentication Use email + SMS code or stronger methods for sensitive cases.
Routing Auto-route to compliance and legal with role-based assignment rules.

Technical Requirements for Secure eSubmission

Choose a platform that supports secure uploads, strong authentication, and a detailed audit trail for every signed questionnaire.

  • File Formats: PDF, DOCX, and common image types accepted.
  • Integrations: Link to CRM or document storage like Salesforce or Google Workspace.
  • Security: TLS in transit and AES-256 at rest required.

Ensure the provider can produce a tamper-evident signed copy with timestamped audit data for regulatory or legal review.

Recommended Timelines and Response Expectations

Use these recommended timelines to preserve evidence and meet typical internal and external reporting needs.

Immediate Acknowledgment:

Acknowledge receipt to the reporter within 24–48 hours.

Preliminary Review:

Complete initial triage within 5 business days.

Investigation Window:

Conduct substantive review and evidence collection within 30 days.

Regulatory Reporting:

Prepare reports within regulatory deadline or as counsel advises.

Case Closure:

Aim to resolve or escalate cases within 90 days where feasible.

Common Mistakes When Preparing a Client Fraud Questionnaire

  • Incomplete identifiers: omitting account numbers or legal names delays matching and resolution efforts.
  • Vague timelines: listing approximate dates rather than exact MM/DD/YYYY entries makes forensic correlation difficult.
  • Missing evidence: failing to attach supporting documents or redacted emails forces repeat collection requests.
  • Unsigned declarations: unsigned or initial-only submissions weaken attribution and may be inadmissible in enforcement actions.

Potential Risks and Consequences of Inaccurate or Late Questionnaires

Regulatory Fines: Civil penalties or enforcement action.
Operational Loss: Delayed mitigation increases financial exposure.
Reputational Harm: Client trust and market standing may decline.
Evidence Gaps: Incomplete records impede prosecution or recovery.
Contractual Breach: Missed notice requirements can trigger defaults.
Insurance Rejection: Claims may be denied for inadequate documentation.

Use Exact Identifiers
Record full legal names, account numbers, and transaction IDs to avoid ambiguity and speed reconciliation across systems.
Require Key Attachments
Make transaction records and communication screenshots required fields to prevent incomplete submissions and reduce investigator follow-up.
