What the Client IC Submission Form Is and when it's used

The Client IC Submission Form is a standard document used to collect information from independent contractors and vendors before they start work for a client. It typically gathers contractor identity, tax and payment details, scope of services, insurance and compliance confirmations, and any required attestations. Organizations use the form to determine classification, enable payments, gather a W-9 or equivalent tax data, check insurance coverage, and set expectations about deliverables and timelines. Proper completion reduces onboarding friction and supports downstream reporting and recordkeeping obligations.

Why this form matters for compliance and payments

A complete Client IC Submission Form centralizes legal, tax, and operational data needed to classify the worker correctly, start payments, and meet reporting duties. It reduces misclassification risk, helps determine whether a W-9 or 1099 applies, and creates a clear record for audits and internal controls.

Why this form matters for compliance and payments

  • Independent Contractors — Provide identification, tax ID, contact and payment instructions.
  • Procurement or Accounts Payable — Verify insurance, payment setup, and classification.
  • Hiring Manager or Project Lead — Confirm scope, deliverables, and acceptance criteria.
