9. REPRESENTATIONS; CERTIFICATION

The Client hereby certifies that the information provided in this Form is true, correct and complete to the best of the Client's knowledge. The Client authorizes Service Provider to verify any information supplied, including but not limited to identity and credit or background checks where permitted by law. The Client agrees to notify Service Provider promptly of any material changes to the information supplied.

10. ENTIRE AGREEMENT

This Client Identification Form, together with any subsequently executed services agreement between the parties, constitutes the entire agreement with respect to the subject matter hereof and supersedes all prior understandings, proposals, representations and agreements, whether written or oral. Any amendment or waiver must be in writing and signed by both parties.
