What the Client Information Form F511 Is and when it's used

The Client Information Form F511 is a standardized intake document used to collect identifying, contact, and engagement details from a new or existing client. It consolidates legal name, mailing and billing addresses, taxpayer identification, primary contacts, emergency or secondary contacts, service preferences, and authorization consents. Organizations use F511 to create client records, verify identity, route communications, and document consent for specific services. Completed forms become part of the client file and may be referenced for billing, compliance checks, and regulatory reporting.

A fully completed Client Information Form F511 reduces onboarding delays, ensures accurate billing and communications, and provides a documented record of client consent.
