What the Client Intake Certification Is

The Client Intake Certification is a formal record completed when an organization documents verification and onboarding data for a new client. It consolidates identity and contact details, conflict checks, scope of services, fee arrangements, and required disclosures into a single certified statement. Organizations use it to confirm they performed required due diligence, established the client relationship, and obtained necessary consents. Contents and execution processes vary by industry and jurisdiction; where electronic signatures are used, compliance with ESIGN and UETA should be ensured to preserve legal enforceability.

Why a Certification Matters for Onboarding and Compliance

A Client Intake Certification documents that required identity checks, conflict screening, disclosures, and consent steps were completed. It creates an auditable record useful for compliance, risk assessment, and client disputes while clarifying responsibilities and the scope of the client relationship.

Why a Certification Matters for Onboarding and Compliance

Who Typically Completes This Certification

Typical users include in-house counsel, intake coordinators, compliance officers, and client-facing professionals responsible for onboarding and recordkeeping.

  • Law firms and solo attorneys performing conflict checks and client onboarding.
  • Healthcare organizations collecting patient consents and HIPAA-related intake information during registration.
  • Financial services and accounting firms verifying client identity and tax reporting details.

Use this form to centralize onboarding steps and create a single point of reference for future audits or client inquiries.

Essential Elements of a Professional Intake Certification

Professional Client Intake Certifications combine standardized fields, verification evidence, attestations, and signature blocks to create a complete onboarding record suitable for audit and compliance reviews.

Identification

Record full legal name, date of birth, driver license or ID number, and current address. Include scanned ID or credential analysis results when required by policy to support identity proofing.

Conflict Checks

Document conflicts of interest search results, dates checked, and who performed the review. Note any restrictions or waivers and retain correspondence that evidences clearance decisions.

Scope & Fees

Describe agreed services, deliverables, estimated timelines, and fee arrangements including retainer amounts, billing cadence, and payment terms. Avoid vague language that could create ambiguity later.

Consents

Capture required client consents for data sharing, background checks, electronic delivery, and payment authorization. For consumer-facing matters, include ESIGN consumer disclosure and documented consent to e-records.

Supporting Evidence

Attach copies of government IDs, W-9 forms when necessary, proof of authorization for business entities, and any third-party verification reports used during intake. Retain source file names and dates received to support audit trails.

Signatures & Dates

Include dated signature blocks for the client and authorized representative, plus printed names and titles. Record the signing method used and capture an audit trail for e-signature events.

Common Preparation Errors to Avoid

  • Incomplete identity information, such as missing date of birth or mismatched legal name, often causes verification failures, delays onboarding, and can trigger additional documentation requests from compliance teams.
  • Using abbreviations for addresses, omitting two-letter state abbreviations, or relying solely on P.O. boxes can impede service of process and create jurisdictional ambiguity.
  • Failing to obtain explicit ESIGN consumer disclosures on consumer-facing transactions risks invalidating electronic consent and may require paper delivery under 15 U.S.C. §7001.
  • Unclear or unsigned fee arrangements, missing retainer details, or vague scope descriptions increase dispute risk and may undermine enforceability in fee disputes.

Tax Penalties: IRC §6721 late-filing penalties possible
Backup Withholding: 24% backup withholding may apply
I-9 Violations: Civil fines $281–$2,789 per violation
