Verify identity and insurance before first visit

Use standardized formats
Enforce MM/DD/YYYY for dates, require full state names or two-letter USPS codes consistently, and validate phone numbers to reduce data normalization work.
Document consent clearly
Present treatment and privacy consent text verbatim and capture dated signatures or electronic acceptance to ensure informed consent and reduce liability.
Secure PHI and log access
Store intake records with encryption at rest, role-based access, and an audit trail to meet HIPAA standards and to support breach investigations if needed.
