What the Client Intake Split Form Is and When It's Used

The Client Intake Split Form documents a new client relationship and records any agreement to split fees, commissions, or payments among multiple parties. It combines client identification, engagement terms, allocation percentages, payment instructions, and authorizations into a single record to ensure transparency among service providers, brokers, or referral partners. Organizations use it to record consent to split arrangements, create a traceable record for accounting and tax purposes, and to capture signatures and contact data necessary for billing, conflict checks, and compliance with industry- and state-specific rules.

Why a Formal Split Form Matters

A properly completed Client Intake Split Form reduces disputes by documenting how revenue is divided, specifies responsibilities, supports accurate tax reporting, and provides an auditable signature record for compliance and internal controls.

Why a Formal Split Form Matters

Who Typically Completes or Signs a Split Form

Front-line staff complete the form during onboarding, and signatories include any party that will receive a portion of fees.

  • Intake coordinators at small and mid-size firms who capture client and split details during onboarding.
  • Brokers and agents who share commissions across teams, offices, or external referral partners.
  • In-house legal or finance teams who review allocation terms for compliance and tax reporting.

Proper role assignment—payer, payee, and witness or notary where required—helps prevent reconciliation errors and supports later audits.

Core Sections Every Professional Client Intake Split Form Should Include

A complete split form balances client identity, allocation math, payment routing, authority to bind, and signature evidence so each party knows rights, obligations, and the administrative steps required to process fees.

Client Identification

Legal name, business name (if applicable), and government ID fields to match tax and billing records; ties the split to the correct taxpayer.

Engagement Summary

Concise description of services or transaction, effective date, and scope so the split applies only to the specified engagement or transaction set.

Split Allocation

Clear percentage or fixed-dollar allocations, calculation examples, and rounding rules to avoid ambiguity in fee distribution and accounting entries.

Payment Instructions

Banking details, payee names, payment method, and timing for each recipient so finance teams can automate disbursements and reconcile invoices.

Authority & Approvals

Representations that signers have authority to bind their organization, conflict-of-interest disclosure, and internal approver names if required.

Signatures & Audit

Signature blocks with dates, witness or notary lines if required, and space for electronic signature metadata and audit-trail references.

Complete the intake form before first billing or service delivery

Payment Schedule:

Specify timing (net 30, net 45) for each recipient

Tax Reporting Trigger:

Annual Reconciliation:

Reconcile split allocations against revenue each fiscal year

Correction Window:

Amend mistakes promptly to reduce audit risk

Common Mistakes to Avoid

  • Unclear allocation math or rounding rules that lead to cumulative rounding discrepancies and payment shortfalls.
  • Entering mismatched taxpayer names or TINs that trigger backup withholding or delayed payments.
  • Failing to capture written authorization from all payees, resulting in disputes over entitlement and reconciliation.
  • Not routing a final signed copy to finance and legal, causing missed payments and audit gaps.

Risks If the Form Is Incomplete or Incorrect

Payment Disputes: Delayed or withheld payments
Tax Withholding: Backup withholding at 24%
Contract Breach: Potential liability for misallocated fees
Privacy Violation: Exposure of PII without proper safeguards
Audit Findings: Missing signatures or records
