The Service Provider's responsibilities include collecting, organizing, and analyzing information provided through this Questionnaire, and preparing summary recommendations as described below.

Confidentiality

Both parties acknowledge that the information provided in this Questionnaire and any materials exchanged in connection with the Services may include confidential and sensitive personal, financial, and medical information. The Service Provider shall maintain such information in confidence and shall not disclose it except (a) as required by law, (b) with the Client's prior written consent, or (c) to third-party service providers engaged to assist in providing the Services who are bound by confidentiality obligations no less protective than those in this Agreement.
