What the Client Merge Signed Form Is and When It’s Used

The Client Merge Signed Form documents the authorized consolidation of duplicate or related client records into a single primary record. It records the identities of the source records, the chosen primary record, the reason for merging, and written authorization from an authorized representative. Organizations use this form to preserve auditability, comply with data governance policies, and create a single source of truth for billing, reporting, and regulatory communications. When executed and retained correctly, the signed form provides a clear trail showing who approved the merge and when the change became effective.

Why a Properly Completed Client Merge Signed Form Matters

A complete, signed merge form reduces data integrity errors, supports regulatory compliance, and documents consent for record changes. It helps avoid billing mistakes, duplicate communications, and disputes about client authority.
