What the Client Onboarding COI Questionnaire Is

The Client Onboarding COI Questionnaire is a standardized form used during onboarding to collect certificate of insurance (COI) details and related insurance metadata from clients, subcontractors, or vendors. It captures insurer names, policy numbers, coverage types, limits, effective and expiration dates, endorsement or additional-insured requests, and any waiver of subrogation requirements. The questionnaire helps onboarding, procurement, and risk teams verify that required coverages match contract terms, identify coverage gaps early, and create a documented record for audit and contract compliance purposes.

Why a COI Questionnaire Matters for Onboarding

A COI Questionnaire centralizes insurance data so teams can confirm compliance with contractual insurance requirements and reduce liability exposure.

Why a COI Questionnaire Matters for Onboarding

Who Uses and Completes This Questionnaire

The questionnaire is typically completed by vendors, subcontractors, or client insurance contacts during onboarding.

  • Procurement teams: collect COIs to meet contractual prerequisites before issuing purchase orders or starting services.
  • Risk and insurance teams: evaluate policy limits, endorsements, and exclusions against contract terms and exposure.
  • Vendors and broker contacts: provide accurate insurer names, policy numbers, effective/expiration dates, and evidence of endorsements.

Responses are reviewed by procurement, risk, contract management, or insurance teams to confirm acceptability.

Who Can Sign or Certify the Questionnaire

Authorized Signatory

A named officer, partner, or authorized representative of the insured who can legally bind the organization. This person confirms the accuracy of insurance details and accepts any attestations on behalf of the entity.

Insurance Coordinator

A broker, agent, or in-house insurance coordinator may complete the form on behalf of the insured when expressly authorized and should include their contact and agency details for verification.

Common Mistakes to Avoid When Preparing a COI Questionnaire

  • Collecting partial policy information, which prevents carrier verification and prolongs onboarding.
  • Using vague language for endorsements like 'as required' without naming exact endorsement forms or wording.
  • Accepting mismatched insured names or DBA listings that do not match the policyholder on file with the carrier.
  • Failing to require a signed attestation or authorized signer, which can invalidate the questionnaire for contractual purposes.

Risks and Consequences of Inaccurate COI Responses

Coverage Gap: Potential uninsured losses
Contract Breach: Failure to meet insurance obligations
Project Delay: Work may be paused pending verification
Financial Liability: Company may absorb claim costs
Regulatory Risk: Fines if regulated coverage missing
Reputational Harm: Loss of client confidence

How COI Questionnaires Differ from Certificate of Insurance Documents

Compare questionnaire-based data collection with traditional issued COIs to choose the appropriate combination for verification and recordkeeping.

Criteria COI Questionnaire Issued Certificate
Purpose collect structured responses carrier-issued proof of insurance
Authorship filled by insured or broker issued by insurer or broker
Verifiability requires carrier check direct carrier document
Use Case onboarding intake and screening final evidence for contract compliance
