What the Client Package Request Form Is

A Client Package Request Form is a standardized document used to collect essential client information and specify which documents or service packages a client needs. It centralizes contact details, package selections, delivery preferences, deadlines, and any required authorizations so teams can assemble and deliver client materials consistently and track requests through completion.

Why a Standardized Request Form Helps Your Process

Using a Client Package Request Form reduces back-and-forth, ensures required data is captured the first time, and creates an auditable record for compliance and billing purposes.

Why a Standardized Request Form Helps Your Process

Who Typically Completes This Form

The form is completed by client-facing staff or the client directly to request a set of documents or services.

  • Account managers and client services teams who prepare onboarding or renewal packages.
  • Clients or authorized representatives submitting requests for records, contracts, or service bundles.
  • Operations or fulfillment teams that receive the request, verify documents, and assemble the package.

Accurate completion assigns responsibility and speeds fulfillment while creating a record useful for audits and dispute resolution.

Essential Sections in a Professional Client Package Request Form

A well-structured form groups related data to reduce errors: requester details, client identifiers, package contents, delivery instructions, required authorizations, and signature confirmation.

Requester Details

Name, department, contact phone and email for clarifying questions and internal routing; helps identify the request originator.

Client Identifiers

Full legal name, date of birth or business EIN, and account number to ensure records match the correct client file.

Package Contents

Predefined package options and a customizable list of documents requested so fulfillment teams can assemble the correct set.

Delivery Options

Preferred delivery method (email, secure portal, physical mail), address or secure account details, and any required handling notes.

Authorizations

Consent, HIPAA or FERPA acknowledgements where applicable, and any third-party release authorizations required to share records.

Signature and Date

Authorized signer name, signature, and effective date confirming the request and permission to proceed.

  • 02

Signature Fields Add signature, date, and initials fields; make signer required
Conditional Logic Show or hide fields based on package type to reduce errors

Digital Delivery, Formats, and Integrations

Confirm platform support for the file types, authentication strength, and downstream integrations your process requires.

  • Integrations: Connectors for Salesforce, NetSuite, Google Workspace, and Microsoft 365 reduce manual entry

Where to Send and How Requests Are Routed
