I agree to notify the Merchant immediately of any change in account information or termination of this authorization. I acknowledge that I am responsible for any fees assessed as a result of returned or rejected payments (including insufficient funds) and for any late fees or collection costs specified by the Merchant in its prevailing terms.

The Merchant may, at its discretion, suspend or terminate services in accordance with its contractual policies if payment is not received when due. Any pre-authorized periodic charges will continue as specified until the end date provided or until terminated in accordance with this authorization.

This authorization is governed by applicable state and federal law. If any provision of this authorization is found invalid or unenforceable, the remaining provisions shall remain in full force and effect. I agree to indemnify and hold the Merchant harmless from all claims, losses or damages arising from my breach of the representations in this authorization.
