Condition Acknowledgement and Release

Client or authorized agent hereby acknowledges receipt of the listed goods and certifies the condition on release as stated above. Upon acceptance and signature, Client releases Service Provider from further custody and liability for damage or loss of the listed goods occurring after the time of pickup, except to the extent caused by Service Provider's gross negligence or willful misconduct.
