What the Client Planning Intake Form Is and When It’s Used

The Client Planning Intake Form collects core client details, objectives, risk tolerances, financial and legal background, and required authorizations to start professional planning services. It centralizes contact data, primary and secondary decision-makers, engagement scope, and consent for data sharing so advisors or firms can assess needs and comply with industry rules. The form is used at client onboarding, before formal recommendations, and when opening accounts, establishing relationships, or initiating fiduciary duties.

Why a Standardized Intake Form Matters for Planning

A consistent Client Planning Intake Form reduces onboarding time, ensures capture of regulatory information, and creates a single source of truth for scope, consent, and recordkeeping. It supports auditability, minimizes missing data, and helps establish timelines and responsibilities between client and provider.

Why a Standardized Intake Form Matters for Planning

Who Typically Completes and Reviews the Intake Form

The intake form is completed by the primary client or authorized representative and reviewed by the planning professional, compliance officer, and account administrator before work begins.

  • Clients and designated representatives complete identification and objectives sections and provide signatures and consents.
  • Advisors or planners verify responses, add internal notes, and confirm scope and fee arrangements.
  • Compliance or legal teams review privacy, data-sharing consents, and required disclosures for regulated industries.

Proper role separation at submission preserves accountability, improves data quality, and reduces downstream rework.

Core Components of a Professional Intake Form

A complete Client Planning Intake Form groups identifiable client data with engagement terms and documentation checkpoints to support analysis, approvals, and secure storage.

Authorizations

Consent for data sharing, account access, fee structure acknowledgement, and e-sign consent where required by consumer-facing statutes.

Typical Submission Path for an Intake Form

A standard online flow reduces handoffs and preserves signed records for compliance and review.
