What the Client Questionnaire Attachment Is and when it’s used

The Client Questionnaire Attachment is a supplemental form appended to a contract or engagement letter that collects standardized client data needed to onboard a relationship, evaluate scope, or support regulatory checks. Typical use cases include collecting contact and business details, authorized signers, billing and tax identifiers, conflict-of-interest disclosures, and industry- or project-specific answers. When completed accurately it reduces follow-up, speeds approvals, and creates an auditable record. The form may be executed electronically under U.S. e-signature law (ESIGN, 15 U.S.C. ch. 96) or state UETA statutes where applicable.

Why include a Client Questionnaire Attachment in your workflow

A standard attachment centralizes critical client data, reduces back-and-forth, and creates a single source of truth for onboarding and compliance. When combined with an e-signature platform it also captures an audit trail and signer attribution without physical filing.

Why include a Client Questionnaire Attachment in your workflow

Who typically completes and relies on this attachment

The Client Questionnaire Attachment is used by teams that must verify identity, gather billing and compliance details, or document contracting preferences before work begins.

  • Sales and account teams collecting contact, billing, and decision-maker details for contract drafting and invoicing.
  • Legal and compliance groups documenting conflict checks, KYC notes, and contract governance choices before execution.
  • Finance and tax teams capturing payee names, TINs, payment terms, and backup withholding instructions for reporting.

Use this form as the initial intake record for procurement, legal review, tax reporting, and secure client communications.

Step-by-step: completing the Client Questionnaire Attachment

Follow these steps to collect information, verify accuracy, and finalize the attachment alongside the primary agreement.

  • 01
Prepare the form: Attach to the main agreement and list required fields for the client.
  • 02
Request completion: Send the attachment to the client with clear instructions and required documents.
  • 03
Verify entries: Compare names, TINs, and addresses against supporting ID or IRS records.
  • 04
Finalize execution: Collect required signatures and store the executed attachment with the contract.

Typical routing and approval flow for the attachment

A standard workflow routes the attachment from requester to client, then to internal reviewers before final execution and storage.

  • Draft: Populate fields and attach to the primary contract.
  • Client input: Client completes required fields and uploads supporting documents.
  • Internal review: Legal and finance verify answers and request clarifications if needed.
  • Execution: Signatures collected and completion recorded in the system.

Core components to include in a professional Client Questionnaire Attachment

A complete attachment balances identification, tax, billing, compliance, and administrative detail so teams can act without follow-up.

Identification

Full legal name, DBA if applicable, legal entity type, and government ID references so on-boarding and KYC checks are straightforward.

Tax information

Taxpayer Identification Number, withholding status, and W-9 or W-8 details to support accurate 1099 or withholding determinations.

Contact details

Primary contact, billing contact, phone number, and email address for contract communications and invoicing workflows.

Payment and invoicing

Preferred payment method, billing cycle, invoice recipient, and remittance address to reduce payment delays.

Compliance disclosures

Conflict-of-interest statements, regulatory status, license numbers, or HIPAA/FERPA notices where industry rules apply.

Execution data

Signature blocks, effective date, witness or notary fields, and document version or attachment identifiers for recordkeeping.

Security and compliance features to document with the attachment

Encryption: TLS 1.2/1.3 in transit; AES-256 at rest
Audit Trail: Timestamps, IPs, and action logs
Certifications: SOC 2 Type II and ISO 27001
HIPAA Support: BAA available where required
21 CFR Part 11: Controls for regulated records
Accessibility: WCAG 2.0 Level AA compliance

Key penalties and risks from incorrect or missing information

1099 Late Penalty: $60–$330 per form (IRC §6721)
Intentional Disregard: $660+ per form, no cap
Backup Withholding: 24% withholding if TIN missing
I-9 Violations: $281–$2,789 per violation (8 CFR §274a.2)
HIPAA Breach: Civil penalties and corrective action
Contract Invalidity: Unsigned or unauthorized execution risks unenforceability

Common mistakes to avoid when preparing the attachment

  • Entering an informal or DBA name instead of the legal entity name, causing mismatches with IRS records and delaying tax reporting.
  • Missing or incomplete taxpayer identification numbers, which can trigger backup withholding and additional verification steps.
  • Failing to list an authorized signer’s title and authority, which can render a signature invalid during contract disputes.
  • Using inconsistent address formats or P.O. boxes where a physical address is required for service or notarization.

Key timing considerations and statutory deadlines

Track these deadlines when the attachment supports tax reporting or regulatory filings; missing dates can create penalties and filing complications.

W-9 Provision Timing:

Provide upon payer request; no fixed federal due date

1099-NEC Deadline:

Recipient and IRS due Jan 31 each year

1099-MISC Deadlines:

To recipient by Jan 31; paper to IRS Feb 28; electronic Mar 31

Individual Tax Return:

Form 1040 due April 15 (Oct 15 with extension)

FBAR Filing:

Due April 15 with automatic extension to Oct 15
