What the Client Referral and Engagement Form Is

The Client Referral and Engagement Form is a structured intake document used to capture referral details, prospective client information, scope of services, and initial engagement terms. It consolidates contact data, referral source, engagement objectives, conflict checks, and consent provisions to establish a clear record before onboarding. Organizations use it to standardize intake, accelerate decision-making, and document authorization for communications and data sharing. When completed and signed, it operates as a formal engagement record that can be retained in client files and integrated with CRM and billing systems for downstream workflows.
