What the Client Review Request Form Is

A Client Review Request Form is a standardized document used to ask a client to review, comment on, or approve deliverables. It records scope, version, review instructions, timelines, and any supporting files so the reviewer and project team share a single source of truth. The form supports electronic signatures and attachments, creating an auditable record of review activity and acceptance consistent with U.S. electronic records laws and commercial workflow practices.

Why use a formal Client Review Request Form

A formal request clarifies expectations, reduces revision cycles, and creates a documented acceptance trail. It centralizes reviewer feedback, timestamps sign-offs, and helps teams meet contractual milestones while preserving a reproducible record of consent.

Why use a formal Client Review Request Form

Who typically completes and processes this form

Project managers, account leads, legal reviewers, and client contacts most often initiate or complete the Client Review Request Form.

  • Project manager coordinating deliverables and deadlines during client review
  • Client representative responsible for technical acceptance or business sign-off
  • Legal or compliance reviewer verifying contract terms or regulatory language

Use role-based routing so the right reviewer sees the correct scope, and maintain version control to avoid duplicate or conflicting review requests.

Technical considerations for digital submission

Common mistakes to avoid when preparing the form

  • Failing to record timestamps and delivery channels, which complicates audits and dispute resolution.
