What a Client Suitability Questionnaire Is

A Client Suitability Questionnaire is a structured intake document used to collect factual, financial, and preference data that helps a professional determine whether a product, service, or investment is appropriate for a prospective client. The questionnaire typically captures identity details, financial situation, investment objectives, risk tolerance, time horizon, liquidity needs, and relevant restrictions. Firms use the responses to comply with suitability and know‑your‑client obligations, to document the decision process, and to create a written record supporting recommendations and risk disclosures when advising or onboarding clients.

Why the Questionnaire Matters for Compliance and Risk Management

A well-designed Client Suitability Questionnaire documents client facts and preferences needed to meet suitability, fiduciary, and AML expectations under U.S. regulatory frameworks.

Why the Questionnaire Matters for Compliance and Risk Management

Who Typically Completes and Reviews This Questionnaire

The questionnaire is completed by the prospective client or an authorized representative and reviewed by the advisor, broker, or intake team.

  • Advisors and brokers review responses to support suitability determinations and recommendations.
  • Private client teams use answers to set risk profiles and investment mandates.
  • Compliance teams retain the record to demonstrate due diligence and audit readiness.

Responses become part of the client file and should be updated whenever material changes occur in the client’s circumstances.

Core Sections to Include in a Professional Questionnaire

A complete Client Suitability Questionnaire organizes information to be consistent, auditable, and defensible. Include identity, financial profile, investment goals, risk tolerance, restrictions, and consent/acknowledgement elements.

Identity

Full legal name, DOB, legal entity type, and government ID details to confirm the signer’s authority and match records.

Financial Profile

Income, net worth, liquid assets, and balance sheet items that affect suitability and ability to bear losses.

Investment Goals

Time horizon, primary objectives (growth, income, preservation), and expected use of proceeds to align recommendations.

Risk Tolerance

Quantified tolerance for volatility and loss, scenario responses, and risk-rating output used to match products.

Restrictions

Legal, tax, regulatory, or personal restrictions such as prohibited investments or tax-sensitive mandates.

Acknowledgements

Consent to electronic records, disclosure of conflicts, and signature block with date and authority statement.

Step‑by‑Step: Completing the Questionnaire

Follow this sequence to collect accurate client information and establish a documented suitability decision.

  • 01
Prepare: Confirm the client’s identity and authorization before sharing the questionnaire.
  • 02
Collect: Send the questionnaire and collect completed answers and supporting documents.
  • 03
Assess: Evaluate responses against risk and suitability policies and document the reasoning.
  • 04
Record: Store the completed questionnaire with audit trail in the client file and note any follow‑up required.

How the Digital Completion Flow Typically Operates

A clear digital workflow reduces friction and preserves an audit trail for each action and signature.

  • Upload: Import the questionnaire PDF or template into the e‑platform.
  • Place Fields: Add required input, checkbox, and signature fields where answers belong.
  • Send: Deliver via email invite or secure signing link with access controls.
  • Capture: Collect signatures, timestamps, and a completion certificate for the record.

Typical Digital Workflow Settings to Configure

Configure these settings to ensure integrity, authentication, and retention of completed questionnaires.

Field Configuration
Authentication Email link or SMS OTP for signer verification
Required Fields Mark identity and financial fields as mandatory
Audit Trail Enable timestamps, IP logging, and activity log
Retention Set automatic archival after retention period

Technical Considerations for Secure eSubmission

Ensure your platform supports secure delivery, authentication, and long‑term storage before accepting electronic completion.

  • Integrations: CRM and document storage connections
  • Formats: PDF, DOCX, and exported audit logs
  • Authentication: Email, SMS, KBA, or SSO options

These technical features help maintain chain‑of‑custody, support compliance audits, and reduce manual reconciliation work.
