What the Client Suitability Review Form Is

A Client Suitability Review Form documents an assessment of whether a proposed engagement, service, or product is appropriate for a specific client based on factors such as financial profile, risk tolerance, regulatory status, objectives, and required disclosures. It collects identifying information, suitability criteria, and the evaluator's conclusions to support compliance and recordkeeping. The form is commonly used by advisors, brokers, legal and compliance teams to demonstrate that a reasoned suitability determination was made and retained in case of audits, disputes, or regulatory review.

Why a Formal Suitability Review Matters

Documenting suitability protects clients and organizations by creating a traceable decision record, meeting regulatory expectations, and reducing disputes. A clear form supports consistent decision-making and shows due diligence during audits or client complaints.

Why a Formal Suitability Review Matters

Who Typically Completes This Form

The Client Suitability Review Form is completed by professionals responsible for assessing client fit, such as financial advisors, compliance officers, and account managers.

  • Financial advisors and brokers assessing investment recommendations and risk tolerance for retail or institutional clients.
  • Compliance officers documenting regulatory review, supervisory sign-off, and escalations for higher-risk matters.
  • Relationship managers or account teams confirming product fit and documenting client objectives and constraints.

Completed forms should be retained according to firm policy and applicable regulations to support audits, disputes, or supervisory review.

Typical Signatory Roles

Advisor

An advisor signs to confirm they collected client facts, applied suitability criteria, and made a recommendation. This narrative should explain the rationale, alternative options considered, and any client-supplied constraints or preferences.

Compliance

A compliance officer or supervisor signs to attest that the review meets internal policy and external regulatory requirements. The note should reference any exceptions granted, mitigations imposed, and required follow-up actions.

Essential Sections to Include on the Form

A professional Client Suitability Review Form organizes client data, suitability criteria, analysis, and sign-off to produce a defensible record. Include the following discrete sections to ensure completeness and auditability.

Client Identity

Full legal name, date of birth, contact information, and taxpayer identification where relevant to confirm the client profile and match records.

Financial Profile

Income, net worth, liquid assets, investment experience, and time horizon to evaluate capacity for loss and product fit.

Objectives & Constraints

Stated objectives (growth, income, preservation), liquidity needs, tax considerations, and any legal or regulatory restrictions affecting recommendations.

Risk Tolerance

Risk questionnaire results, qualitative notes on client attitudes toward volatility, and any stress-test outcomes or scenario analysis performed.

Suitability Analysis

Clear explanation of why the recommended product or service is appropriate, covering risks, alternatives considered, and tradeoffs.

Approvals & Date

Signatures, printed names, titles, dates, and a field for supervisor or compliance approval with reference numbers for tracking.
