What the Client Unblocking Form Is and when it applies

A Client Unblocking Form documents a request to remove a hold, suspension, or restriction placed on a client account, service, or access. It records the requester, the reason for the block, evidence or verification steps completed, and the approval or denial decision. Typical uses include restoring financial account functionality, re-enabling user access to systems, lifting compliance holds, or resolving administrative suspensions. The form creates an auditable record of the unblock decision, any conditions attached to reinstatement, and the date the client regained access.
