What a Clinical Travel Assignment Form Is and when it's used

A Clinical Travel Assignment Form documents the temporary placement of a clinician or clinical staff member at a location away from their primary employer or usual worksite. It records assignment dates, host facility details, scope of clinical duties, licensing and credential verification, pay and reimbursement terms, and any travel-related allowances. The form creates a clear record for the sending agency, receiving facility, and clinician to manage liability, billing, credentialing and payroll for the assignment period, and supports auditing and regulatory retention requirements.

Why a clear Clinical Travel Assignment Form matters

Using a standardized form reduces ambiguity about responsibilities, clarifies pay and travel reimbursement, and documents licensing and credential checks required for clinical practice.

Why a clear Clinical Travel Assignment Form matters

Who typically completes and signs this form

Ensure each party retains a signed copy and that signatory authority aligns with organizational delegation policies.

  • Sending Employer — Human resources or staffing coordinator completes clinician employment, benefits, and payroll data.
  • Receiving Facility — Credentialing or site manager confirms scope of work, supervision, and facility access requirements.
  • Clinician / Contractor — Signs to accept assignment terms, travel reimbursements, and confirms license and insurance information.

Typical signers and their roles

Staffing Manager

The staffing manager or HR representative executes on behalf of the sending organization; they confirm employment status, salary continuation or temporary pay rates, and authorize travel budgets.

Site Administrator

The receiving facility's administrator or credentialing officer signs to accept the clinician, specify duties and supervision, and confirm insurance and liability arrangements for the assignment period.

Key components to include in a professional form
