COMPENSATION AGREEMENT ADDENDUM

This Compensation Agreement Addendum (the Addendum) is entered into as of by and between Client Name: and Employer Name: . The Client and the Employer are sometimes referred to herein collectively as the Parties and individually as a Party.

RECITALS
