The Owner authorizes the condominium association and its designated management company to update association records, deliver statements and notices to the updated contact information, and to remit or accept assessment payments per the banking instructions provided herein. The Owner acknowledges that any changes to banking or payment instructions may require verification and up to thirty (30) days to become effective for billing and remittance processes.

The Management Representative certifies that, if submitting on behalf of an owner, the Representative has written authorization to act and will provide proof of such authorization upon request. Management agrees to maintain confidentiality of unit owner financial information, to follow reasonable verification procedures before accepting or redirecting funds, and to notify the Owner and association in the event of suspected fraud or irregularity.

Remedies & Indemnity: The submitting party agrees to indemnify and hold the Association and Management harmless from liabilities, losses or damages arising from inaccurate information or unauthorized changes submitted by the party. The Association’s acceptance of this form does not waive its right to pursue remedies for unpaid assessments, breaches of governing documents, or fraud.

Governing Law: This form and any dispute arising from it shall be governed by the laws of the state in which the condominium is located, without regard to principles of conflict of laws. This form constitutes the complete written instruction for the updates requested and supersedes prior update requests for the same subject matter unless explicitly stated otherwise.
