INSURANCE & INDEMNITY

Applicant agrees to defend, indemnify, and hold harmless Issuing Authority, its officers, agents, and employees from any claims, liabilities, damages, costs or expenses arising out of Applicant's performance of the drilling operations, except to the extent caused by the gross negligence or willful misconduct of Issuing Authority.

PAYMENT TERMS

Permit fee and related charges shall be paid in accordance with the schedule below. Failure to timely pay fees may result in revocation of this permit.

This permit authorizes work only during the period specified below and may be suspended or revoked for noncompliance with permit conditions.

Information submitted in connection with this permit that is identified by Applicant in writing as confidential shall be treated as such to the extent permitted by law. Applicant acknowledges that certain information may be subject to public disclosure under applicable public records laws and that Issuing Authority makes no guarantee of confidentiality beyond legal requirements.

GOVERNING LAW

This Permit shall be governed by and construed in accordance with the laws of the jurisdiction of Issuing Authority. Any dispute arising under or related to this Permit shall be venued in the appropriate court of that jurisdiction.

ENTIRE AGREEMENT

This Permit, together with any written conditions or attachments expressly incorporated herein, constitutes the entire agreement between Applicant and Issuing Authority with respect to the permitted work and supersedes all prior agreements and understandings, whether written or oral.
