What the Construction Request Authorization Form Is

A Construction Request Authorization Form is a written record used to request, authorize, and document discrete work on a construction project. It captures scope, location, estimated cost, scheduling, required permits, insurance acknowledgements, and approvals from the owner or authorized representative. The form serves as a control point for budgeting, scheduling, and compliance; when completed it creates an auditable record that contractors, subcontractors, and project managers use to proceed with work and to trigger billing or change-order workflows.

Why this form matters for project control

The Construction Request Authorization Form provides a single, auditable record of approval for discrete tasks, reducing misunderstandings, scope creep, and payment disputes while preserving necessary compliance details.
