What the Consulting Information Form Is and When it’s Used

A Consulting Information Form collects key details about an engagement between a consultant and a client, including contact information, scope, billing details, tax identifiers, and authorization to proceed. It functions as an intake and recordkeeping document that supports contracting, invoicing, and regulatory reporting. Organizations use it to capture payer data for 1099 reporting, confirm client billing contacts, document project milestones, and establish governing law and payment terms before starting work.

Why a Structured Consulting Information Form Matters

A consistent form reduces onboarding delays, improves tax compliance, and creates an auditable record of client authorization and billing terms. It also clarifies responsibilities and supports clear invoicing and 1099 reporting when required.

Why a Structured Consulting Information Form Matters

Typical Users and Where This Form Fits

Consultants, project managers, and finance teams use this form during client onboarding to capture facts needed for contracting, billing, and tax compliance.

  • Independent consultants capturing client legal name, billing address, and taxpayer identification for 1099 reporting.

  • Finance or accounts payable teams using the form to validate payee data and payment instructions.

Use the form early in the engagement cycle—before work begins or invoices are issued—to avoid rework, payment delays, and reporting errors.
