What the Contractor Work Request Form Is and When It Applies

A Contractor Work Request Form is a standardized document used to request, specify, and authorize work to be performed by a contractor or subcontractor. It captures requester details, contractor identity and license, scope of work, schedule, cost estimates, required permits or insurance, and signature blocks for approvals. Organizations use this form to document authorization before work begins, create a record for invoicing and lien waivers, and establish responsibilities and acceptance criteria for project closeout.

Why a Formal Work Request Improves Project Controls

Using a consistent Contractor Work Request Form reduces ambiguity, speeds approvals, and creates an auditable record for payments, change orders, and compliance with licensing or insurance rules.

Why a Formal Work Request Improves Project Controls

Who Typically Prepares and Receives This Form

Recipients include contractors, subcontractors, accounting for payment, and recordkeeping systems for audit and retention.

  • Project managers and site supervisors who schedule and control on-site work and budgets.
  • Procurement or purchasing teams issuing purchase orders and verifying contractor credentials.
  • Facilities managers and property owners who require documented approvals and insurance verification.

Step-by-Step: Completing and Routing the Work Request

Follow a clear sequence to minimize rework: complete fields, verify credentials, route for approval, and capture signatures.

  • 01
Prepare Request: Fill all required fields and attach supporting documents.
  • 02
Verify Credentials: Check license, insurance, and W-9 or tax ID.
  • 03
Route for Approval: Send to approvers in the specified order with due dates.
  • 04
Record Signatures: Collect signatures and save the completed form in records.

Essential Sections to Include in a Professional Form

  • E-sign Platform: Use an eSignature service with audit trail and storage.
  • RON / Notary: Required when legal acknowledgement or notarization is necessary.
