This Contribution Payment Notice constitutes a demand for immediate payment of the Total Amount Due. If full payment is not received by the Due Date indicated above, the outstanding balance shall accrue interest at the lesser of 1.5% per month or the maximum rate permitted by applicable law, calculated monthly and compounded. A late fee equal to the greater of $25 or 5% of the overdue amount may be applied on the first day following the Due Date.

In the event of default, the Issuer reserves the right to suspend membership privileges, assess collection costs, recover reasonable attorneys' fees and court costs, and pursue any remedies available under the contribution agreement, bylaws, or applicable law. Contributor agrees that Issuer may offset any amounts payable to Contributor against delinquent contributions where permitted by agreement or law.

Disputes regarding the amount billed must be submitted in writing to the Issuer within fifteen (15) days from the Notice Date above, including specific grounds and supporting documentation. Absent timely written dispute, the amount is deemed final and undisputed.
