Consent for Adoption – This is a consent signed by the person, such as a parent or guardian, who is relinquishing rights with regard to a minor, for the purposes of adoption by a named person(s) in the state of Alabama. This consent is declared to be voluntary and irrevocable. It must be signed and acknowledged before a notary.
