What a Coordination Assistance Request Is and When It’s Used

A Coordination Assistance Request is a formal written request used to ask another organization or internal team to coordinate services, resources, or information for a client, patient, or project. Typical uses include arranging cross-agency support, transferring case responsibility, requesting specialist services, or organizing multi-party logistics. The form identifies the requester, the recipient, the scope of coordination, timeframes, and authorizations needed to share information or act on behalf of the requester. Many organizations accept electronically signed requests under U.S. e-signature law when the document meets intent, consent, attribution, and retention requirements.

Why a Clear Coordination Assistance Request Matters

A well-prepared request reduces delays, clarifies responsibilities, and documents consent to information sharing and task assignments. It creates a single reference that downstream teams can use to track actions and evidence authorization. Accurate forms improve auditability and support regulatory compliance when sensitive data is involved.

Why a Clear Coordination Assistance Request Matters

Who Typically Completes and Responds to These Requests

Organizations use Coordination Assistance Requests when multiple parties must act together or when controlled data sharing requires documented consent and task assignment.

  • Case managers and social workers coordinating services across providers and agencies, ensuring tasks are assigned and timelines are clear.
  • Clinical staff and health information managers requesting specialty consultations, records transfers, or care coordination while meeting privacy rules.
  • Program administrators and contracting officers arranging vendor tasks, scheduling inspections, or requesting contractor deliverables.

Use the form to record who requested assistance, who will perform it, what information may be shared, and any deadlines or constraints that apply.

Where to Send the Completed Request and What Happens Next

Routing depends on organizational policy; use the recipient contact noted on the form and follow documented escalation paths for urgent matters.

  • Email Submission: Send to the official intake mailbox for logging and assignment.
  • Online Portal: Upload to the designated case management or service portal.
  • Interagency Fax: Use secure, signed fax only where permitted by policy and law.
  • In-Person Handoff: Deliver to the recipient's office and obtain a stamped receipt.

Delivery Options and Digital Signing Considerations

Choose distribution channels that meet your confidentiality and audit requirements when sharing the request and attachments.

  • Unclear or open-ended scope forces multiple clarifying exchanges and lengthens resolution time.
  • Submitting unsupported file formats or oversized attachments can prevent intake systems from ingesting evidence.

Risks and Consequences of an Incorrect or Incomplete Request

Service Delay: Delayed approvals
Denial of Request: Request rejected
Data Breach Risk: Unauthorized disclosure
Regulatory Noncompliance: Potential fines
